Send documents to confirm date of birth

If you get a notice from the Marketplace that says you need to send documents with your child’s date of birth to confirm that you’re eligible for a Special Enrollment Period, you need to send the Marketplace confirmation of the new child’s date of birth. Send a document for each new child, if there’s more than one, and your notice asks for documents for more than one person.

IMPORTANT: Upload or mail documents as soon as possible. If you don’t submit documents by the deadline, you won’t have Marketplace coverage.

What documents can I submit?

  • Copy of the birth certificate or application for a birth certificate for the child.
  • Copy of the Social Security card for the child.
  • Copy of the application for a Social Security Number (SSN) for the child.
  • Copy of the medical record from a clinic, hospital, physician, midwife, institution, or other medical provider showing the child’s date of birth.
  • Letter from a hospital, clinic, physician, or other medical provider attesting the child’s date of birth.
  • Copy of the military record showing the child’s date and place of birth.
  • Copy of a foreign birth record showing the child’s date and place of birth.
  • Copy of a religious record showing the child’s date and place of birth.
  • Letter or other document from the health insurance company, like an “Explanation of Benefits”, showing that services related to birth or post-birth care were provided to the child the mother, or both, including the dates of service.

You can upload more than one document if you have multiple documents to confirm date of birth. Select the green “UPLOAD DOCUMENTS” button on the upload screen in your application to submit each document.

What if I don’t have any of these documents?

You can submit a letter providing the child’s birth date and the reason you can’t provide documents. The Marketplace will take your letter into consideration. To submit this letter, select “Letter of explanation” from the drop-down menu when you’re on the upload screen in the application.
