More information for people with special health care needs

Share:

A person may qualify to get more health services if he or she has special health care needs. If the person pays for care, he or she may also qualify to pay less. Special health care needs include if a person:

  • Has a medical, mental health, or substance abuse condition that limits his or her ability to work or go to school
  • Needs help with daily activities, like bathing or dressing
  • Regularly gets medical care, personal care, or health services at home or in another community setting, like adult day care
  • Lives in a long term care facility, group home, or nursing home
  • Is blind
  • Is terminally ill

If a person has any of these special health care needs, and wants to see if he or she qualifies, let us know by checking the box next to that person’s name.
