The Marketplace will send you Form 1095-A, “Health Insurance Marketplace® Statement” for each person covered by a Marketplace plan during the year you’re filing taxes for. Form 1095-A will help you enter information on your tax form about the premium tax credit (also called a subsidy, discount, or savings).
- Keep your Form 1095-A with other tax records, like the W-2 you may get from your employer.
- Make sure the information about your premium is correct, as described below.

Is your Form 1095-A correct?
See Part III of your form for information about health plan premiums.
Column A: your monthly enrollment premium It may be different from your plan’s full monthly premium amount. This doesn’t always mean there are errors that need to be corrected. The monthly premium on your 1095-A may be different from what you expect because:
- Your plan included benefits in addition to the essential health benefits required by the health care law, like adult dental or vision benefits. In this case, the monthly enrollment premium on your Form 1095-A may show only the amount of your premium that applied to essential health benefits.
- You or a household member started or ended coverage mid-month. In this case, your Form 1095-A will show only the premium for the parts of the month coverage was provided.
- You were enrolled in a stand-alone dental plan and a dependent under 18 is enrolled in it. In this case, the monthly enrollment premium listed on your Form 1095-A may be higher than you expect because it includes a portion of the dental plan premiums for pediatric benefits.
If any of these applied to you, the premium shown on your Form 1095-A is probably correct. If you still think the premium shown in Column A is wrong, contact the Marketplace Call Center.
Column B: your second lowest cost Silver plan (SLCSP) premium information You should see amounts for each month any household member had Marketplace coverage.
Here are 2 examples of SLCSP information that isn’t correct:
- If Part III, Column B has a “0” or is blank for any month someone in your household had the Marketplace plan.
- If you had changes to your household that you didn't report to the Marketplace, like having a baby, moving, getting married or divorced, or losing or gaining a dependent. In this case the numbers in Column B may be wrong.
If either of these applies to you, you’ll need to get the correct SLCSP premium. You can use a tax tool if you need to fill in missing or corrected information on your Form 1095-A. Get more information on how to use the tax tool.
Once you have a complete and accurate Form 1095-A, you’ll be ready to file your taxes and find out the amount of the premium tax credit you qualify for based on your income.
